During martial law, the consent of the second parent may not be required; however, the final requirements are determined by the border guard service, so we still recommend having this document.   • a military registration document, either in paper form or in the “Reserve+” application, containing up-to-date information, including for men aged 18–22.
The following documents are required to arrange medical insurance:
  • The first page of the international passport. If the documents are available in the “Diia” application, screenshots of the opened passport must be provided.
  • Tax identification number, which is mandatory for adults but not required for children. If there is no identification number, prepare the first and second pages of the Ukrainian passport, the page confirming refusal of a tax identification number, and photos of both sides of the ID card.
